Qualifications:
Coordinates implementation and tracking of projects / programs with adherence to guidelines and targeted populations. Under general direction of Director, provides professional nursing services to clients in their homes or clinic settings using acquired nursing skills to identify health related problems, implement appropriate intervention measures and promote standard health practices. Participates in community health programs to promote wellness of individuals. Duties and Responsibilities: 1. Provides nursing services including diagnosis by assessment of client's needs, development of health related goals; provision of client care; referral to other agencies and health care providers; maintains client records according to agency policy, refers to and consults with physician in planning appropriate care; ensures client safety and security; reports observed or suspected instances of child or adult abuse pursuant to mandated requirements; provides instruction and supervision of Home Care Aide providing services to clients. 2. Writes patient and family health goals in order to provide a systematic health care plan for the improvement of health by reviewing patient/family assessment data, including patient and family interviews, conferring with medical staff and determining appropriate health care goals and methods, communicates regularly with physicians, family members, pharmacists and other professionals in the provision of program services. 3. Writes reports for agency administration and program evaluation by compiling statistics, conducting audits of health care records and documenting work activities. 4. Attends continuing education classes required to maintain R.N. license. 5. Follows established policies, protocols and procedures of Webster County Public Health. Qualifications: This position requires RN licensure and a current, valid Iowa driver's license.
